The International Committee of Medical Journal Editors has a document Uniform Requirements for Manuscripts Submitted to Biomedical Journals: Writing and Editing for Biomedical Publication and the NLM has a page of sample bibliographic citations in the recommended uniform format. This uniform format is supported by QuikBib so can be used as an export format from both Scopus and RefWorks, among others. Should all manuscript submission systems accept (and publish) this format (thus doing away with the hundreds of different citation formats), or is it not sufficient for covering citations in the humanities and other subject areas?
